Job summary

PeaceHealth is seeking a Speech Pathologist for Home Health in Florence, Oregon. This full-time, 1.0 FTE role offers a day shift with competitive pay and relocation/hiring bonus potential. You will evaluate patients in their home environments, establish treatment plans, and collaborate with a multidisciplinary team to achieve optimal outcomes.

Requires Oregon state licensure as a Speech-Language Pathologist, CCC-SLP, and BLS. Experience in home health is preferred.

Qualifications

• Master's or Doctorate Degree in Speech and Language Pathology.
• Licensed Speech Pathologist in Oregon (OSHP).
• CCC-SLP certification required.
• Basic Life Support (BLS) certification.

Responsibilities

• Evaluates patients and establishes a plan of care with defined goals.
• Delivers treatment and updates care plan as needed.
• Completes accurate documentation per regulatory standards.
• Collaborates with healthcare professionals to ensure continuity of care.
• Mentors and supervises clinical fellows and therapy aides.

PeaceHealth is seeking a Speech Pathologist Home Health for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at PeaceHealth is $46.27 – $69.41. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.

Our multi-disciplinary Home Health team consists of Physical Therapists, Occupational Therapists, Speech Language Pathologists, RN’s, Medical Social Workers, Registered Dietitians and Bath Aides. Job Summary

Responsible for the evaluation of patients, determining plan‑of‑care and goals of treatment, participating in treatment of patients within the scope of Speech and Language Pathology. Provides oversight to conditional licensees in the clinical fellowship year and students, in following the plan‑of‑care and treatment of patients appropriate to the age of the patient served; provides supervision to therapy aides and volunteers. Works as part of a patient care team toward the best outcomes for the patient.

What you will do

• Evaluates assigned patients and establishes a treatment plan and goals, using data from a variety of sources, such as patient records, interviews, observation and team members.
• Completes patient treatment according to plan of care and modifies plan to meet patient needs, and updates plan as appropriate.
• Completes documentation accurately, timely, according to regulatory and state licensure requirements, and within professional standards.
• Collaborates with other healthcare professionals as necessary to ensure smooth continuous care of the patients. May act as a liaison with nursing, medical and other therapy staffs to facilitate problem solving and coordination of other services and act as an educational resource.
• Mentors and provides oversight to new caregivers, conditional licensees in the clinical fellowship year, and therapy aides. Provides clinical instruction and training to students.
• Recommends and participates in development of new services and programs, quality improvement and monitoring activities.
• Participates in staff meetings, patient care meetings, educational in‑services, caregiver orientation and other professional activities.
• Performs other duties as assigned.

What you bring

Education

• Master's or Doctorate Degree Required: Speech and Language Pathology accredited by the American Speech and Hearing Association

Experience

• Requires 1 year of experience in Home Health.

Credentials

• Required: Licensed Speech Pathologist - Oregon
• Required: Cert of Clinical Competence in Speech‑Language Pathology
• Required: Upon Hire Basic Life Support through American Red Cross or American Heart Association
• Requires valid driver's license and proof of insurance.
