Job summary

LiveWell Homecare Agency, LLC is seeking a Speech-Language Pathologist to evaluate and treat patients with speech, language, cognitive-communication, and swallowing disorders during home visits. You will design evidence-based treatment plans and provide individualized therapy.

You will document patient outcomes in the EMR, collaborate with an interdisciplinary team, educate patients and families about progress, and assist with discharge planning and program development to enhance in-home care.
