Sans Souci Rehabilitation & Nursing Center in Harrison, NY is seeking a Speech Language Pathologist (SLP) to provide and direct Speech Therapy services, including assessment, treatment planning, and interdisciplinary collaboration under physician orders.

The SLP will conduct screenings, evaluate patients, develop individual treatment plans, coordinate with families and staff, monitor progress, and document results.

Responsibilities

• Provides and directs Speech Therapy services to patients, including assessment and treatment.
• Conducts screenings of patients at regular intervals to determine need for intervention.
• Evaluates patients to obtain data for treatment planning and implementation.
• Interprets and communicates evaluation findings to patients, family, and team members.
• Establishes individual patient schedules with other team members.
• Develops treatment plans with goals and methods to achieve them.
• Coordinates treatment plan with patient, family and other team members.
• Monitors progress and modifies treatment as indicated.
• Recommends and uses equipment to improve patient communication; trains patient and staff on devices.
• Participates in discharge planning and develops home or community programming.
• Documents results of assessment, treatment, follow-up, and termination of services.
• Supervises students and volunteers per state regulations.
• Maintains ongoing continuing education and professional development.
• Records indirect and billable treatment times and submits statistics.
