Job summary

Casselman Healthcare & Rehabilitation Center in Meyersdale, PA is seeking a Speech Therapist to assess and treat patients requiring rehabilitation. The role involves direct patient care, developing individualized treatment plans under physician direction, and documenting patient progress.

Responsibilities include evaluations, progress notes, education for patients and families, and collaboration with the care team to deliver resident-centered therapy while maintaining confidentiality and HIPAA

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of NY.
• Experience with long-term care and/or geriatric care is preferred but not limited to.

Responsibilities

• Assess referred patients that require rehabilitation services.
• Provide direct patient care to assess medical condition, functional capabilities, limitations and rehab potential.
• Establish and administer a treatment program with goals based on patient capacity under physician direction.
• Set realistic and achievable goals for patients.
• Document and record the patient’s condition.
• Educate patients and families in appropriate therapy methods.
• Complete evaluations, weekly progress notes, recertification and discharge documents.
• Collaborate with all disciplines to provide best patient-centered care.
• Perform all duties assigned by Director of Rehabilitation.
• Ensure confidentiality and compliance with HIPAA and Federal Residents Rights.
