Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Tiffin, OH. This full-time role delivers comprehensive speech therapy services to residents.

You will assess patients, develop treatment plans with goals, educate families and staff, and ensure CMS-compliant documentation in NetHealth EMR.

Qualifications

• Master's degree in Speech Language Pathology is required.
• Certificate of Clinical Competency is required.
• Active SLP license in state(s) of practice is required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services in SNF.
• Evaluate and obtain data necessary for treatment planning.
• Develop a treatment plan with target list, short-term and long-term goals.
• Instructs patient, family, and staff in speech-language procedures for carryover in daily activities.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to ensure resident needs are met timely and with clinical excellence.

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency

Active SLP license in state(s) of practice

Ongoing continuing education and professional development to maintain licensure and certification
