Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist to provide in-house services for a Skilled Nursing Facility in Silvis, IL. The role involves evaluating residents, developing treatment plans, and delivering therapy to support resident communication and swallowing needs.

Candidates must hold a Master's degree in Speech-Language Pathology, CCC, and an active SLP license in the relevant states. Proficiency with NetHealth EMR and CMS guidelines is essential.

Qualifications

• Master's degree in Speech Language Pathology.
• CCC (Certificate of Clinical Competency) required.
• Active SLP license in practicing state(s).
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data for treatment planning.
• Develop a treatment plan with target list, short-term, and long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to meet resident needs timely with clinical excellence.
