Job summary

Evolve Therapy Services is hiring compassionate Speech Language Pathologists for an in-house Skilled Nursing Facility in Silvis, IL. Responsibilities include planning, developing, and directing SLP services to meet CMS and regulatory standards, ensuring the highest quality resident care.

The role requires a Master’s in Speech-Language Pathology, CCC, and an active license. Competitive pay with full-time, part-time, or PRN options and CMS-compliant documentation experience are expected.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification

Responsibilities

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education

Master's degree in Speech Language Pathology
