Job summary

Evolve Therapy Services in Silvis, IL is seeking a Speech Language Pathologist for our in-house Skilled Nursing Facility. The role supports planning, organizing, developing, and directing SLP services to ensure high-quality resident care.

We offer Full Time, Part Time, or PRN positions. Must have CCC and active SLP license with ongoing CEU development. Responsibilities include therapy delivery, data collection, treatment planning, CMS documentation in NetHealth EMR, and FEES/MBS coordination.

Qualifications

• Master's degree in Speech Language Pathology required.
• Certificate of Clinical Competency (CCC) required.
• Must hold an active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan with target list, short-term and long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversee and implement FEES/MBS protocols with site coordination.
• Participate in screening protocols to determine resident needs and ensure timely care.
