Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist to join our partner site in Franklin, MN, providing in-house therapy services for a Skilled Nursing Facility. The role includes comprehensive evaluation, treatment planning, and interdisciplinary collaboration to ensure high-quality resident care.

The position offers full-time, part-time, or PRN options with a $5,000 sign-on bonus and tuition reimbursement.

Qualifications

• Master's degree in Speech Language Pathology.
• CCC required.
• Active SLP license in applicable state(s).
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and collect data for treatment planning and implementation.
• Develop treatment plans with target lists and goals.
• Instruct patients, families, and staff on speech-language procedures.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to meet resident needs timely.
