Job summary

Evolve Therapy Services, LLC is seeking a licensed Speech Language Pathologist to support an in-house skilled nursing facility in Franklin, MN. The role involves delivering quality evaluations and therapies, creating treatment plans, and coordinating with residents, families, and staff.

Qualifications include a Master’s in Speech Language Pathology, CCC, and an active SLP license in the state. Full-time, part-time, or PRN options available with tuition reimbursement offered.

Qualifications

• Must hold a Master's degree in Speech Language Pathology.
• CCC certification required for eligibility to practice.
• Active SLP license in the state(s) of practice is required and ongoing CEIs encouraged.

Responsibilities

• Provide quality speech therapy services following established standards and CMS guidelines.
• Evaluate and gather data for treatment planning and implementation.
• Develop treatment plans with target lists and short/long-term goals.
• Instruct patients, families, and staff in speech-language procedures for carryover.
• Complete compliant CMS documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening to identify residents' needs and deliver timely care.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

Full Time (offering tuition reimbursement!), Part Time, or PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
