Evolve Therapy Services seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility, in a PRN telehealth/remote role. You will plan, supervise, and deliver Speech Language Pathology services to residents per federal, state, and local standards.

The role requires a Master’s in SLP, CCC-SLP, and an active license; ongoing CE to maintain licensure is expected. Competitive pay with benefits applies to full-time status where available.

Qualifikationen

• Must hold CCC-SLP and active license in state(s) of practice.
• Experience with FEES/MBS protocols is a plus.
• Telehealth/remote work ability and professionalism required.

Aufgaben

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with targets, short- and long-term goals.
• Instruct patients, families and staff to ensure carryover of therapy goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocol implementation and coordination.
• Participate in resident screening to identify needs and deliver timely, quality care.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: PRN (Telehealth - Remote)

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
