Evolve Therapy Services is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Wisconsin. This PRN Telehealth role focuses on evaluating residents, developing treatment plans, and delivering quality speech therapy services in accordance with CMS and state guidelines.

You will document in NetHealth EMR, supervise FEES/MBS protocols, coordinate with site teams, and pursue ongoing professional development to maintain licensure and CCC.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency (CCC).
• Active SLP license in required states.
• Commitment to ongoing continuing education and licensure maintenance.

Responsabilités

• Provide quality speech therapy services and initiate referrals per established standards.
• Evaluate and obtain data necessary for treatment planning and progress.
• Develop a treatment plan with target list, short- and long-term goals.
• Instruct patient, family, and staff in speech-language procedures to promote carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to meet residents' needs with clinical excellence.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: PRN (Telehealth - Remote)

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
