Job summary

Gallatin Center for Rehabilitation and Healing is seeking a Speech Language Pathologist (SLP) to join our team, full-time with PRN options. We offer a sign-on bonus and highly competitive rates.

The SLP will assess patients requiring rehabilitation, develop and implement treatment plans, and document patient progress under physician direction. Responsibilities include setting goals, educating patients and families, and collaborating with interdisciplinary teams to ensure exceptional

Qualifications

• Must have graduated from an accredited Speech-Language Pathology program.
• Must hold a valid TN state license/registration.
• Experience in long-term care and/or geriatric settings preferred but not required.

Responsibilities

• Assess referred patients needing rehabilitation services.
• Establish and administer a tailored treatment program with specific goals.
• Set realistic goals based on patient capacity and document progress.
• Educate patients and families in therapy approaches and home programs.
• Collaborate with all disciplines to provide patient-centered care.
• Maintain timely documentation including evaluations, progress notes, and discharge summaries.
• Adhere to confidentiality, HIPAA, and Residents' rights.
