Job summary

Gallatin Center for Rehabilitation and Healing is seeking a Speech Language Pathologist (SLP) to join our team. This role offers a sign-on bonus and highly competitive rates for both full-time and PRN positions.

The SLP will assess referred patients requiring rehabilitation, establish treatment plans, document progress, and educate patients and families. Tennessee licensure is required, with preferred experience in geriatric care and long-term care settings.

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must hold valid TN licensure and registration.
• Experience with long-term care or geriatric populations preferred.
• Strong documentation and record-keeping skills.
• Ability to work with interdisciplinary teams.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care and determine treatment plans.
• Set realistic goals based on patient tolerance and capacity.
• Document patient condition, progress notes, and discharge plans.
• Educate patients and families on therapy methods and expectations.
• Collaborate with all disciplines to deliver patient-centered care.
