Job summary

Gallatin Center for Rehabilitation and Healing seeks a Speech Language Pathologist (SLP) to join our care team. The role includes assessing referred patients, developing and administering a targeted treatment plan, and documenting progress under physician direction.

The SLP will set realistic goals, educate patients and families, and collaborate with all disciplines to deliver patient-centered care. This full‑time position (with PRN options) offers a sign‑on bonus and highly competitive rates.

Qualifications

• Graduated from an accredited Speech-Language Pathology program.
• Must have valid license and TN registration.
• Experience with long-term care and/or geriatric care preferred.
• Strong documentation writing.
• Includes evaluations, weekly progress notes, recertification and discharge documentation.
• Collaborates with all disciplines to provide patient-centered care.

Responsibilities

• Assess referred patients requiring rehabilitation services and determine capacity for rehab.
• Establish and administer treatment programs with goals set by patient tolerance and physician direction.
• Document patient condition and progress; educate patients and families.
