Job summary

Shadelands Post Acute in Walnut Creek, CA, is seeking a compassionate Speech Language Pathologist (SLP) for a PRN position with weekend availability. You will provide comprehensive speech, language, cognitive, and swallowing services to help patients reach their highest function.

Responsibilities include developing treatment plans, evaluating patients, coordinating with physicians and the interdisciplinary team, and documenting care in a timely manner.

Qualifications

  • Active Speech Language Pathologist license.
  • Experience with patient assessment, treatment planning, and clinical documentation.
  • Strong communication and interpersonal skills.
  • Ability to work on an interdisciplinary healthcare team.

Responsibilities

  • Develop individualized treatment plans and obtain physician authorization.
  • Provide speech-language pathology services per orders and plans.
  • Evaluate patients and set goals based on clinical findings.
  • Monitor progress and communicate changes to supervisors and team.
  • Participate in care conferences and rehabilitation meetings as needed.
  • Complete evaluations, treatment notes, progress notes, and discharge documentation timely.
  • Document treatment changes per policies and payer requirements.
  • Educate patients, families, caregivers, and staff on strategies and maintenance.
  • Support discharge planning and transitions of care.
  • Assist with staff education and clinical development.
  • Maintain compliance with board regulations and facility policies.
  • Build positive relationships with patients and colleagues.

Responsibilities include:* Develop individualized treatment plans and obtain appropriate physician authorization for services.* Provide speech-language pathology services in accordance with physician orders and established treatment plans.* Evaluate patients and develop appropriate goals and interventions based on clinical findings.* Monitor and communicate patient progress, concerns, and changes in condition to supervisors, physicians, and interdisciplinary team members.* Participate in Patient Care Conferences, Utilization Review Meetings, Rehabilitation Conferences, and other care coordination activities as needed.* Complete evaluations, daily treatment notes, progress notes, recertifications, discharge documentation, and other required records accurately and timely.* Document treatment changes and modifications in accordance with facility policies, professional standards, and payer requirements.* Provide education and training to patients, families, caregivers, and nursing staff regarding therapeutic strategies, maintenance programs, and safe swallowing practices.* Support effective discharge planning and transitions of care.* Assist with staff education, in-service training, and clinical development programs.* Maintain compliance with applicable Speech-Language Pathology and Audiology Board regulations, state practice acts, facility policies, and professional standards.* Build positive, respectful relationships with patients, families, coworkers, and healthcare professionals.Qualifications* Current and active **Speech Language Pathologist (SLP) license*** Strong clinical, communication, and interpersonal skills* Ability to work effectively as part of an interdisciplinary healthcare team* Experience with patient assessment, treatment planning, and clinical documentation* Compassionate, professional, and patient-centered approach* Strong organizational skills and attention to detailMake a Difference With UsAt Shadelands Post Acute, we are committed to providing compassionate, high-quality care in a supportive environment.
