Job summary

Elara Caring in Connecticut seeks a PRN Speech Language Pathologist to provide home health services in the Middlebury and Danbury area. You will evaluate patients, develop plans of care, and deliver therapy per physician orders.

You will educate patients and caregivers, document accurately, and collaborate with an interdisciplinary team; travel between patient homes required; valid SLP license, CT licensure, and a Master’s degree are required.

Qualifications

• Master’s degree in Speech Language Pathology.
• Current and unrestricted SLP license in the state of practice (CT).
• At least one year of clinical experience as an SLP.
• Valid driver’s license or reliable transportation for home visits.

Responsibilities

• Evaluate patients and develop individualized plans of care.
• Provide speech and language therapy services according to physician orders.
• Help patients improve their communication, cognitive, and swallowing abilities.
• Educate patients and caregivers on home therapy programs.
• Maintain accurate and timely clinical documentation.
• Follow OASIS, PPS, and company guidelines.
• Collaborate with physicians and interdisciplinary team members to ensure continuity of care.
