Job summary

Elara Caring in Massachusetts is seeking a PRN Speech-Language Pathologist to evaluate and treat patients in their homes, focusing on voice, language, cognition, and swallowing.

You will develop treatment plans, educate patients and caregivers, document clinically, and coordinate with an interdisciplinary team; traveling between patient homes is required with a valid license and ability to lift up to 50-100 lbs.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current, unrestricted Speech-Language Pathologist license in the state of practice.
• At least one year of SLP experience in a clinical care setting.
• Valid driver’s license or reliable transportation to travel between patient homes.

Responsibilities

• Evaluate patients’ speech, language, cognitive, communication, and swallowing needs.
• Develop individualized treatment plans based on physician orders and assessment findings.
• Provide skilled therapy services in patients’ homes.
• Teach patients and caregivers exercises, techniques, and strategies to support continued progress.
• Monitor patient progress and update treatment plans as needs change.
• Complete accurate and timely clinical documentation.
• Coordinate services with nurses, therapists, caregivers, and other healthcare professionals.

PRN Home Health Speech-Language Pathologist (SLP)

What You’ll Do

• Evaluate patients’ speech, language, cognitive, communication, and swallowing needs.
• Develop individualized treatment plans based on physician orders and assessment findings.
• Provide skilled therapy services in patients’ homes.
• Teach patients and caregivers exercises, techniques, and strategies to support continued progress.
• Recommend and provide instruction on assistive devices when appropriate.
• Monitor patient progress and update treatment plans as needs change.
• Communicate changes in condition, progress, and recommendations to physicians and the interdisciplinary care team.
• Coordinate services with nurses, therapists, caregivers, and other healthcare professionals.
• Complete accurate and timely clinical documentation.
• Maintain compliance with professional standards, regulatory requirements, and company policies.

What You’ll Need

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current, unrestricted Speech-Language Pathologist license in the state of practice.
• At least one year of SLP experience in a clinical care setting.
• Valid driver’s license and an acceptable driving record, or reliable transportation to travel between patient homes and assigned locations.
• Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.

Pay is based on a per-point value with a range of $65-$75 per point.
