Job summary

Elara Caring is seeking a PRN Speech Language Pathologist to serve patients in the Middlebury and Danbury area. You will evaluate patients, develop individualized plans of care, and provide therapy in accordance with physician orders.

This role emphasizes meaningful one-on-one patient care, home therapy education, and coordination with an interdisciplinary team. You will need a Master's in Speech Language Pathology, active SLP license, and at least one year of experience.

Qualifications

• Master's degree in Speech Language Pathology is required.
• Current and unrestricted SLP license in the state of practice.
• At least one year of clinical experience as a Speech Language Pathologist.
• Must possess a valid driver's license or reliable transportation for travel between patient homes.
• Ability to travel throughout the Middlebury and Danbury coverage area.
• Ability to lift, push, pull and support up to 50 pounds.

Responsibilities

• Evaluate patients and develop individualized plans of care.
• Provide speech and language therapy services according to physician orders.
• Help patients improve communication, cognitive, and swallowing abilities.
• Educate patients and caregivers on home therapy programs.
• Maintain accurate and timely clinical documentation.
• Follow OASIS, PPS, and company guidelines.
• Collaborate with physicians and interdisciplinary team members to ensure continuity of care.

What We're Looking For

• Master's degree in Speech Language Pathology
• Current and unrestricted SLP license in the state of practice
• At least one year of clinical experience as a Speech Language Pathologist
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to travel throughout the Middlebury and Danbury coverage area
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
