Job summary

FutureCare in Baltimore is seeking a licensed Speech Language Pathologist to join an interdisciplinary team. You will assess residents, develop individualized treatment plans, and document progress to improve speech, language, and cognitive communication.

Qualifications

  • Master’s or Doctoral Degree in Speech-Language Pathology.
  • Valid Maryland clinical license for SLP.
  • ASHA Certification required.
  • Demonstrated competency in applying SLP principles to residents.
  • Ability to assess resident needs and develop a care plan.
  • Knowledge of regulatory standards; CFY welcome.

Responsibilities

  • Evaluate resident information on assessment and treatment impact.
  • Conduct and document thorough resident assessments and diagnoses.
  • Develop and implement individualized treatment programs with functional goals.
  • Comply with facility evaluation, treatment and documentation guidelines.
  • Document treatment in resident medical records and discharge summaries.
  • Revise treatment programs based on progress.
  • Orient residents and families to services and potential risks.
  • Collaborate with nursing staff and families to maximize resident function.
  • Inform residents and caregivers about procedures post-discharge.
  • Assist referrals and stay informed about community resources.

Salary Range $46.50 - $52.92/HR

Salary Disclosure Statement

The salary mentioned above reflects the potential base pay range for this role. Bonuses or other incentives (if applicable) are offered separately and paid pursuant to the relevant program schedule. All employment offers will consider factors such as overall experience, job-related qualifications, location, certifications/training, etc.

Responsibilities
  • On referral, evaluates the impact of available resident information on the assessment and treatment process
  • Conduct and document a thorough assessment of the resident to determine and diagnose communicative deficits and equipment needs
  • Develop and implement an individualized treatment program based on the initial evaluation and containing achievable functional goals
  • Comply with evaluation, treatment and documentation guidelines described in facility policy and procedures
  • Complete appropriate documentation in the resident’s medical record following each treatment session
  • Revise the resident’s treatment program, as needed, based upon resident performance
  • Document the course of treatment in a clear, concise discharge summary utilizing a prescribed format
  • Orient residents to program services and treatment procedures appropriate to their needs. Inform residents of any potential risk during any procedure
  • Work with nursing staff, residents and families to maximize residents’ use of functional abilities
  • Instruct residents and family/caregiver about therapeutic procedures to be continued by the resident following discharge
  • Assist in the referral process when a resident requires additional services following discharge. Maintain current knowledge of community resources
Qualifications
  • Graduate of an approved program in Speech/Language Pathology culminating in the receipt of a Master’s or Doctoral Degree
  • Valid license in Speech Language Pathology services at the clinical level in the state of Maryland
  • Certification by the American Speech-Language-Hearing Association
  • Demonstrated competency in applying the principles, methods, materials and equipment used in Speech/Language Pathology to the resident population served in this facility
  • Ability to assess resident needs and develop and implement a comprehensive plan of care
  • Knowledge of regulatory standards and compliance requirements
  • SLP Clinical Fellowship is welcome
