Job summary

Sigma Health Rehab is seeking a per diem Speech Language Pathologist to join Holliswood Center for Rehabilitation and Nursing in Hollis, NY. The role involves providing evaluations, developing treatment plans, and delivering patient-centered therapy in a state-of-the-art rehab setting.

The ideal candidate will hold a NYS SLP license, graduated from an accredited SLP program, and be available for per diem shifts, including evenings and weekends. Mentorship and ongoing clinical support are offered.

Qualifications

• Graduate of an accredited SLP program.
• Current New York State SLP license.

Responsibilities

• Administer Speech Therapy evaluations & examinations to diagnose residents & develop treatment plans.
• Monitor residents' progress and adjust treatments accordingly.
• Record information on the initial evaluation, treatment, progress, and discharge of clients.
• Observe and assess the progress & prepare detailed reports.
• Maintain resident confidence by monitoring confidential information processing.
• Ensure compliance with relevant laws, regulations, or standards.
• Maintains a cooperative relationship among healthcare team members & contributes to a team effort.
