Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist (SLP) for an in-house Skilled Nursing Facility, offering PRN telehealth support.

The role involves planning, delivering, and coordinating speech-language pathology services to residents in a SNF setting, ensuring CMS-compliant documentation and high-quality care.

Qualifications

• Must hold an active SLP license in the practicing state(s).
• Master's degree in Speech-Language Pathology required.
• Certificate of Clinical Competency (CCC-SLP) required.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide quality speech therapy services for residents in the facility.
• Evaluate data for treatment planning and implement therapy plans.
• Develop treatment goals and monitor progress with residents and families.
• Instruct patients, families, and staff in speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening to identify resident needs and ensure timely care.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: PRN - Telehealth

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
