Evolve Therapy Services in Rochester, MN seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility. This PRN role supports evaluation, treatment planning, and direct therapy to residents with comprehensive documentation in NetHealth EMR.

The ideal candidate holds a Master's in Speech Language Pathology, CCC, and an active SLP license in the state, with ongoing commitment to licensure maintenance and professional development.

Qualifications

• Master's degree in Speech Language Pathology required.
• Certificate of Clinical Competency (CCC) required or eligible.
• MUST have active SLP license in state(s) of practice.
• Commitment to ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services in an in-house SNF and initiate referrals as appropriate.
• Evaluate data for treatment planning and implement goals.
• Develop treatment plans with short-term and long-term objectives.
• Educate patient, family, and staff to generalize techniques.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversight of FEES/MBS protocols and site coordination.
• Participate in resident needs screening ensuring timely, clinical care.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility
Job Type: PRN
Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.
Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.

• To evaluate and obtain data necessary for treatment planning and implementation

• To develop a treatment plan that includes a target list, short-term, and long-term goals

• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities

• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems

• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs

• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology

• Certificate of Clinical Competency

• MUST have active SLP license in state(s) of practice.

• Ongoing continuing education and professional development to maintain current licensure and certification
