Job summary

Evolve Therapy Services is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility in Minneapolis, MN. This PRN position focuses on delivering quality therapy, evaluating needs, and developing targeted plans to improve resident communication and feeding safety.

You will document in NetHealth EMR, coordinate FEES/MBS protocols, and work with residents, families, and staff to ensure treatment goals are achieved with clinical excellence.

Qualifications

• Master's degree in Speech Language Pathology required.
• CCC/Credentials required for clinical practice.
• Active state license required for practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with goals and timelines.
• Instruct patients, families, and staff to ensure carryover of therapy goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening to ensure need fulfillment with clinical excellence.

Education

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.

• To evaluate and obtain data necessary for treatment planning and implementation

• To develop a treatment plan that includes a target list, short-term, and long-term goals

• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities

• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems

• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs

• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology

• Certificate of Clinical Competency

• MUST have active SLP license in state(s) of practice.

• Ongoing continuing education and professional development to maintain current licensure and certification
