Job summary

Evolve Therapy Services in Phelps, KY is seeking a compassionate Speech Language Pathologist for an in-house Skilled Nursing Facility. This part-time or PRN role offers opportunities to impact resident care within a collaborative therapy team.

You will provide quality speech therapy, evaluate data for treatment planning, develop short- and long-term goals, instruct residents, families and staff, and ensure compliant documentation in NetHealth EMR, including CMS expectations.

Qualifications

• Master’s degree in Speech Language Pathology required.
• CCC certification or eligibility required.
• Active SLP license in state(s) of practice required.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with short- and long-term goals.
• Instruct patient, family, and staff to carry over remediation
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident screening to identify needs and ensure timely care.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

Part Time or PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.

• To evaluate and obtain data necessary for treatment planning and implementation

• To develop a treatment plan that includes a target list, short-term, and long-term goals

• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities

• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems

• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs

• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology

• Certificate of Clinical Competency

• MUST have active SLP license in state(s) of practice.

• Ongoing continuing education and professional development to maintain current licensure and certification
