Evolve Therapy Services is seeking a Speech Language Pathologist to join our in-house skilled nursing facility team in Kentucky. The role focuses on providing high-quality therapy, planning treatments, and coordinating with staff to support resident care.

Qualifying candidates will hold a Master’s in Speech Language Pathology with CCC and an active state license. This part-time/PRN position offers flexible hours and opportunities for professional growth within the organization.

Qualifications

• Must hold a Master’s degree in Speech Language Pathology and CCC.
• Active SLP license in the practice states is required.
• Commitment to ongoing continuing education and licensure

Responsibilities

• Provide quality speech therapy services in a skilled nursing facility.
• Evaluate data for treatment planning and implement goals.
• Develop and monitor treatment plans with short- and long-term targets.
• Educate patients, families, and staff to ensure carryover of therapy.
• Complete CMS-compliant documentation in NetHealth EMR.
• Ensure FEES/MBS protocols are followed with proper coordination.
• Participate in resident screening to meet needs timely and with care.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

Part Time or PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
