Job summary

Evolve Therapy Services in Madisonville, TN is seeking a compassionate Speech-Language Pathologist for our partner in an in-house Skilled Nursing Facility. This is a part-time or PRN role focused on delivering high-quality resident care.

You will evaluate patients, develop treatment plans, provide patient/family education, document in NetHealth EMR, and ensure CMS compliance. Active SLP licensure and CCC are required.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan that includes a target list, short-term, and long-term goals.
• Instruct patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities.
• Complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems.
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs.
• Participate in screening protocols for establishing needs of residents timely and with clinical excellence.

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions
