Job summary

Evolve Therapy Services is seeking a compassionate Speech Language Pathologist for an in-house Skilled Nursing Facility in Marion, Ohio. This full-time role involves planning, organizing, and directing speech-language services in accordance with applicable standards and regulations under the Administrator's direction.

You will deliver quality therapy, evaluate data for treatment planning, develop goals, train patients, families, and staff, and document in NetHealth EMR per CMS requirements.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan that includes targets, short-term, and long-term goals.
• Instructs patient, family, and staff in speech-language procedures to ensure carryover.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversee FEES/MBS protocols and coordination of site programs.
• Participate in screening protocols for resident needs.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Full Time

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.

• To evaluate and obtain data necessary for treatment planning and implementation

• To develop a treatment plan that includes a target list, short-term, and long-term goals

• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities

• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems

• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs

• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology

• Certificate of Clinical Competency

• MUST have active SLP license in state(s) of practice.

• Ongoing continuing education and professional development to maintain current licensure and certification
