Job summary

Evolve Therapy Services, LLC is hiring a Speech Language Pathologist to join an in-house Skilled Nursing Facility in Randallstown, MD on a PRN basis. You will assess, diagnose, and treat residents, develop treatment plans, and provide family and staff training to ensure continuity of care.

Responsibilities include data collection for treatment planning, maintaining compliant documentation in NetHealth EMR, and collaborating with the care team to optimize resident communication and swallowing

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop treatment plans with target lists, short-term and long-term goals.
• Instruct patients, families, and staff for carryover of remediation and goals.
• Complete compliant CMS-mandated documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident screenings to identify needs and ensure timely care.

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• MUST have active SLP license in state(s) of practice.
