Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist to support an in-house Skilled Nursing Facility in South Pasadena, FL. This PRN position focuses on delivering expert speech-language pathology services and coordinating patient care.

The role requires CCC, active state licensure, and ongoing professional development; responsibilities include assessments, treatment planning, CMS documentation, and collaboration with residents, families, and staff.

Qualifications

• Master’s degree in Speech-Language Pathology.
• CCC certification or eligibility.
• Must hold an active SLP license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services per standards and productivity.
• Evaluate and collect data for treatment planning and implementation.
• Develop treatment plans with short- and long-term goals.
• Instruct patients, families, and staff to ensure carryover of therapy.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols with site coordination.
• Screen residents to identify needs and ensure timely care.

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications
