Job summary

Evolve Therapy Services, a therapy management organization, is seeking a compassionate Speech Language Pathologist for an in-house Skilled Nursing Facility in Clarion, PA. This PRN position supports resident care and therapy services.

You will assess and treat residents, develop treatment plans with clear goals, document in NetHealth EMR, and coordinate FEES/MBS protocols with the site team. Ongoing licensure requirements and CCC certification are required.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency (CCC).
• Active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and referrals per standards.
• Evaluate and obtain data for treatment planning.
• Develop treatment plans with target list and goals.
• Instruct patient, family, and staff for carryover of therapy.
• Complete compliant documentation in NetHealth EMR per CMS.
• Oversight of FEES/MBS protocols and site program coordination.
• Participate in screening to meet resident needs with clinical excellence.

Job Position

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
