Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist to provide in-house services at a Skilled Nursing Facility in Pahokee, Florida. This part-time/PRN role focuses on assessment, treatment planning, and targeted communication goals with residents.

Responsibilities include data collection, FEES/MBS coordination, CMS-compliant documentation in NetHealth, and collaboration with staff, residents, and families. Requires CCC and active SLP license.

Qualifications

• Master's degree in Speech Language Pathology is required.
• Certificate of Clinical Competency (CCC) is required.
• Active SLP license in state(s) of practice is required.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan with target list, short-term and long-term goals.
• Instruct patient, family, and staff in speech-language procedures.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to identify residents' needs.

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
