Evolve Therapy Services, a therapy management organization, seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility in Pahokee, FL. This part-time or PRN position focuses on delivering high-quality speech therapy and coordinating care with the care team.

Qualifications

• Master's degree in Speech-Language Pathology is required.
• Certificate of Clinical Competency is required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan with targets and short/long-term goals.
• Instruct patients, families, and staff in speech-language procedures to ensure carryover.
• Complete compliant documentation in CMS expectations within NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to identify residents' needs in a timely manner.

Job Type

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence
