Evolve Therapy Services is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Florida. This Part Time or PRN position focuses on delivering high-quality speech therapy services to residents and coordinating care with interdisciplinary teams.

The job requires a Master's degree in Speech Language Pathology, CCC, and an active SLP license in the state. CMS-compliant documentation in NetHealth EMR is expected, with opportunities for growth.

Qualifications

• Master's degree in Speech Language Pathology is required.
• Certificate of Clinical Competency (CCC) is required.
• Must hold an active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and collect data for treatment planning and progress.
• Develop treatment plans with short- and long-term goals.
• Instruct patients, families, and staff to apply speech-language techniques in daily activities.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee and implement FEES/MBS protocols with site coordination.
• Participate in screening protocols to ensure timely resident needs are met with clinical excellence.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

Part Time or PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.

• To evaluate and obtain data necessary for treatment planning and implementation

• To develop a treatment plan that includes a target list, short-term, and long-term goals

• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities

• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems

• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs

• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology

• Certificate of Clinical Competency

• MUST have active SLP license in state(s) of practice.

• Ongoing continuing education and professional development to maintain current licensure and certification
