Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist to provide in-house therapy at a Skilled Nursing Facility in Sutherland, NE. The role emphasizes compassionate care, comprehensive evaluation, and evidence-based treatment planning.

Responsibilities include data collection, interdisciplinary collaboration, and CMS-compliant documentation in NetHealth EMR, with oversight of FEES/MBS protocols and resident screening to meet clinical excellence standards.

Qualifications

• Must hold a Master’s degree in Speech Language Pathology.
• CCC certification required for clinical practice.
• Active SLP license in state(s) of practice is mandatory.

Responsibilities

• Provide quality speech therapy services and initiate referrals per standards.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists and short/long-term goals.
• Instruct patient, family, and staff on speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR as required.
• Oversee FEES/MBS protocols and site program coordination.
• Participate in screening to ensure timely resident needs and clinical excellence.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
