Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist to join our in-house skilled nursing facility team in Pikesville, MD. The role focuses on planning, developing, and directing speech-language pathology services under CMS guidelines and with hospital partnerships.

The successful candidate will deliver quality therapy, collect data for treatment planning, and coordinate with staff to ensure goals are met while maintaining compliance with regulatory standards.

Qualifications

• Master's degree in Speech-Language Pathology required.
• CCC required for certification eligibility and ongoing licensure.
• Active SLP license in state(s) of practice is mandatory.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with targets and short/long-term goals.
• Instruct patient, family, and staff to enable carryover of therapy goals.
• Complete CMS-compliant documentation in NetHealth EMR.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Full Time

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
