Job summary

Evolve Therapy Services, LLC is hiring a compassionate Speech Language Pathologist for an in-house Skilled Nursing Facility in Clarion, PA. This PRN position supports resident care and therapy services.

The role requires a Master’s degree in Speech-Language Pathology with CCC and an active SLP license. You will evaluate residents, develop treatment plans, document in NetHealth EMR, and coordinate with staff to ensure quality care.

Qualifications

• Master's degree in Speech Language Pathology required.
• CCC (Certificate of Clinical Competency) required.
• Active state licensure as an SLP in practicing states.
• Commitment to ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data necessary for treatment planning.
• Develop treatment plans with target lists and short/long-term goals.
• Instruct patients, families, and staff in SLP procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening to ensure timely resident needs are met.

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long‑term goals
• Instructs patient, family, and staff in appropriate speech‑language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
