Job summary

Evolve Therapy Services is seeking a compassionate Speech Language Pathologist to join our in-house skilled nursing facility team in Prince Frederick, MD. You will assess, plan, and implement speech-language pathology services in accordance with federal, state, and local standards and resident care goals.

Responsibilities include providing quality therapy, data collection for treatment planning, creating short- and long-term goals, and documenting CMS-compliant progress in NetHealth EMR.

Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• Active SLP license in state(s) of practice
• Ongoing continuing education and professional development to maintain licensure and certification

Responsibilities

• Provide quality speech therapy services in accordance with standards and department procedures.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists and goals (short-term and long-term).
• Instruct patient, family, and staff in speech-language procedures for carryover into daily activities.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening to identify resident needs in a timely manner.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Full Time, Part Time, or PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
