Job summary

Evolve Therapy Services is seeking a compassionate Speech-Language Pathologist for an in-house Skilled Nursing Facility in Phenix City, Alabama. This PRN position focuses on evaluating and treating residents with communication and swallowing disorders, delivering therapy per standards, and documenting in NetHealth EMR.

The role emphasizes client-family education, collaboration with site staff, and adherence to CMS guidelines.

Qualifications

• Master's degree in Speech Language Pathology required.
• CCC credential preferred and highly valued.
• Active SLP license in the practitioner's states of responsibility.

Responsibilities

• Provide quality speech therapy services and initiate referrals per standards and CMS requirements.
• Evaluate and collect data for treatment planning and implementation.
• Develop treatment plans with target lists and short/long-term goals.
• Instruct patients, families, and staff to carry over therapy goals in daily activities.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to meet resident needs timely and with clinical excellence.

Job Position: Speech Language Pathologue for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
