Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Lamberton, MN. The role offers full time, part time, or PRN options to provide compassionate, high-quality therapy services.

You will plan, organize, and direct Speech Language Pathology services ensuring compliance with CMS and state regulations, while collaborating with administrators and care teams to maintain resident care quality.

Qualifications

• Must have a Master's degree in Speech Language Pathology.
• CCC-SLP certification or eligibility is required.
• Active state license to practice SLP in applicable states is required.

Responsibilities

• Provide quality speech therapy services in accordance with standards and productivity guidelines.
• Evaluate and collect data for treatment planning and implementation.
• Develops individualized treatment plans with short- and long-term goals.
• Instructs patients, families, and staff on speech-language procedures for carryover.
• Complete compliant documentation in NetHealth EMR as required by CMS.
• Oversee FEES/MBS protocols and coordinate site programs.
• Screen residents to determine needs and ensure timely care with clinical excellence.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Full Time, Part Time, or PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
