Job summary

Evolve Therapy Services is seeking a compassionate Speech Language Pathologist for an in-house Skilled Nursing Facility in Oklahoma City, OK. This PRN role focuses on delivering quality therapy, developing treatment plans, and ensuring compliance with CMS expectations within NetHealth EMR systems.

Responsibilities include direct therapy, data collection for planning, educating patients and staff, and coordinating FEES/MBS protocols to meet resident needs with clinical excellence.

Qualifications

• Master's degree in Speech-Language Pathology required.
• Certificate of Clinical Competency required.
• Must have active SLP license in state of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals per standards.
• Evaluate and collect data for treatment planning and implementation.
• Develop treatment plans with goals (short-term and long-term).
• Instruct patients, families and staff to ensure carryover of therapy goals.
• Complete compliant documentation according to CMS expectations (NetHealth EMS).
• Oversee FEES/MBS protocols and coordinate site programs.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.

• To evaluate and obtain data necessary for treatment planning and implementation

• To develop a treatment plan that includes a target list, short-term, and long-term goals

• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities

• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems

• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs

• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology

• Certificate of Clinical Competency

• MUST have active SLP license in state(s) of practice.

• Ongoing continuing education and professional development to maintain current licensure and certification
