Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility partner in Duluth, MN. The role covers evaluation, treatment planning, goal setting, and direct therapy to enhance communication and swallowing function.

Responsibilities include CMS-compliant documentation in NetHealth EMR, coordinating with staff and families, and ensuring high standards of resident care. Flexible options include full-time, part-time, or PRN assignments.

Qualifications

• Master's degree in Speech Language Pathology.
• CCC-SLP certification and an active state license.
• Active licensure in state(s) of practice required.
• Commitment to ongoing licensure maintenance and CE.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate, in line with standards and CMS expectations.
• Evaluate and obtain data for treatment planning and implementation.
• Develop a treatment plan including target lists with short- and long-term goals.
• Instruct patients, families and staff in speech-language procedures for carryover in daily activities.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to ensure needs of residents are met promptly and with clinical excellence.

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency

MUST have active SLP license in state(s) of practice

Ongoing continuing education and professional development to maintain current licensure and certification

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Full Time, Part Time, or PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
