Evolve Therapy Services, LLC is hiring a Speech Language Pathologist for an in-house skilled nursing facility in Memphis, TN. The role is full-time with a $3,000 sign-on bonus, or PRN as alternative.

You will plan, organize, and direct speech pathology services, ensuring care meets federal, state, and CMS requirements. The position emphasizes high-quality resident care, documentation, and collaboration with the care team.

Qualifikationen

• Must have a Master’s degree in Speech Language Pathology and be CCC
• Active SLP license in state(s) of practice
• Ongoing continuing education to maintain licensure and certification

Aufgaben

• Provide quality speech therapy services and initiate referrals as appropriate
• Evaluate and obtain data for treatment planning and implementation
• Develop treatment plans with target lists and short-/long-term goals
• Instruct patients, families, and staff in speech-language procedures for carryover
• Complete compliant documentation per CMS expectations in NetHealth EMR
• Oversee FEES/MBS protocols with site coordination
• Participate in screening protocols to meet resident needs timely and with clinical excellence

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

Full Time ($3,000 sign on!) or PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
