Job summary

OneWell Health Care is seeking a compassionate Speech-Language Pathologist (SLP) to provide personalized therapy to individuals with IDD in home and community-based settings in Pennsylvania.

You will assess communication and swallowing needs, develop treatment plans, and collaborate with families and professionals to improve communication, independence, and quality of life. This per-diem, part-time role offers flexible scheduling and rapid onboarding in Elizabethtown, PA.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current and valid Speech-Language Pathologist license in the state of practice.
• CCC-SLP certification from ASHA.
• Strong clinical, communication, documentation, and organizational skills.
• Valid driver's license, reliable transportation, and willingness to travel to home and community-based service locations.
• Experience supporting individuals with Intellectual and Developmental Disabilities (IDD), providing home or community-based services, and working with AAC or dysphagia management is preferred.
• English proficiency is required; bilingual proficiency, including Spanish, is a plus.

Responsibilities

• Conduct speech, language, cognitive-communication, AAC, and swallowing evaluations.
• Develop and update individualized treatment plans.
• Provide evidence-based speech, communication, AAC, feeding, and swallowing therapy.
• Collaborate with families, caregivers, and interdisciplinary team members.
• Educate caregivers on communication strategies, AAC use, and safe swallowing practices.
• Complete accurate and timely clinical documentation.
• Participate in required meetings and trainings.
