Job description

BAYADA Home Health Care

Currently seeking an experienced Speech Language Pathologist, SLP for home health visits in Franklin County, PA.

Prior home care experience is a plus but not required. As an SLP, you will be part of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to receive necessary medical care at home.

Responsibilities:

1. Make home visits to clients in designated areas.
2. Perform evaluations to assess, diagnose, and treat speech, language, communication, or swallowing issues.
3. Develop and revise treatment plans based on evaluations, physician’s orders, and goals.
4. Participate in multidisciplinary care planning.
5. Educate patients and families on rehabilitative activities to promote health, safety, and independence.
6. Document observations and interventions accurately using electronic tablets.

Qualifications:

• Current PA Speech Language Pathologist license.
• Certificate of clinical competence from ASHA, or equivalent education and experience, with certification exam pending, or in process of accumulating supervised work experience.
• Strong computer skills; EMR experience preferred.
