Job description

BAYADA Home Health Care

is currently seeking an experienced Speech Language Pathologist (SLP) for a per diem opportunity performing home health visits for our office located in Gaithersburg, MD.

Our office services adult and geriatric patients in homes throughout Washington County. Prior home care experience as an SLP is a plus but not required.

As a home care Speech Language Pathologist, you will be an integral member of a multidisciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care while remaining at home.

Responsibilities:

1. Make home visits to clients in designated geographic areas.
2. Perform evaluations to assess, diagnose, and treat clients with speech, language, communication, or swallowing difficulties.
3. Develop and revise treatment plans based on evaluations, physician's orders, and goals.
4. Participate in multidisciplinary care planning with BAYADA clinicians.
5. Educate patients and family members on rehabilitative care and activities to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using a touch pad tablet.

Qualifications:

1. Current license as a Speech Language Pathologist in MD.
2. Certificate of Clinical Competence from the American Speech-Language-Hearing Association (ASHA), or equivalent educational credentials and work experience, with ongoing certification exam eligibility.
3. Strong computer skills; experience with electronic medical records (EMR) preferred.
