BAYADA Home Health Care is currently seeking an experienced Speech Language Pathologist, SLP, for a per diem opportunity performing home health visits for our office located in Rock Hill, SC. This office services adult and geriatric patients in homes throughout Lancaster, Chester, and York Counties.

Prior home care experience as a Speech Language Pathologist, SLP is a plus, but not required.

As a home care Speech Language Pathologist, SLP, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities:

1. Make home visits to clients in designated geographic territories.
2. Perform evaluations to assess, diagnose, and provide therapeutic interventions for clients with speech, language, communication, or swallowing difficulties.
3. Develop a treatment plan based on evaluations, physician’s orders, and goals.
4. Assess and revise the speech therapy care plan; participate in multidisciplinary care planning.
5. Educate patients and family members on rehabilitative care and activities to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using electronic medical records.

Qualifications:

1. Current license as a Speech Language Pathologist in SC.
2. Certificate of clinical competence in speech from the American Speech and Hearing Association, or equivalent education and work experience, and eligibility for certification.
3. Solid computer skills; prior EMR experience preferred.
