Job description

BAYADA Home Health Care is seeking a Speech Language Pathologist (SLP) for a per diem home health position. This role involves providing home health visits to adult and geriatric patients across Greenville, Spartanburg, and Anderson counties.

As a home care Speech Language Pathologist, you will be a vital part of a multidisciplinary healthcare team delivering rehabilitative and skilled nursing care, enabling patients to receive necessary medical services while remaining at home.

Responsibilities include:

1. Making home visits within designated geographic areas.
2. Conducting evaluations to diagnose and treat speech, language, communication, or swallowing issues.
3. Creating and updating individualized treatment plans based on evaluations, physician’s instructions, and patient goals.
4. Participating in multidisciplinary care planning and adjusting therapy plans as needed.
5. Educating patients and families on rehabilitative activities to promote health, safety, and independence.
6. Documenting patient observations, interventions, and evaluations using electronic tablets.

Qualifications include:

1. Current South Carolina Speech Language Pathologist license.
2. Certificate of Clinical Competence from the American Speech-Language-Hearing Association (ASHA), or equivalent educational and work experience, with eligibility to take the ASHA certification exam.
3. Strong computer skills; experience with electronic medical records (EMR) preferred.
