Speech Language Pathologist (SLP) – Part-time/Per Diem

VNA & Hospice of the Southwest Region is seeking compassionate and motivated Speech Language Pathologists (SLPs) to join our team in Rutland County. We are hiring for part-time and/or per diem positions working with adult and/or pediatric patients in the home health setting.

Position Summary

As an SLP, you will provide skilled speech therapy services to patients in the home, facility, or educational environment as part of an interdisciplinary care team. The SLP evaluates patients, develops individualized treatment plans, and provides therapeutic interventions to address communication disorders, swallowing difficulties, developmental delays, and cognitive-linguistic impairments resulting from injury, illness, disability, or developmental conditions.

Key Responsibilities
  • Performs comprehensive (OASIS and SLP) assessments of assigned patients including, but not limited to psychosocial, educational, safety and risk factor parameters, as evidenced by clinical documentation.
  • Actively manages and adjusts patient schedules to meet productivity goals and patient care needs, including coordinating schedule changes and communicating updates to supervisors.
  • Performs initial and ongoing assessments of patients' communication, cognitive, developmental, and/or swallowing status.
  • Develops and implements individualized treatment plans based on physician orders and patient needs.
  • Provides direct speech, language, voice, fluency, cognitive, and swallowing therapy services in the patient's home.
  • For pediatric patients, supports speech/language development, feeding and swallowing skills, and caregiver education.
  • For adult patients, addresses communication, cognition, and swallowing deficits related to stroke, neurological conditions, injury, or illness.
  • Instructs and educates patients, caregivers, and family members regarding goals and home exercise programs.
  • Collaborates with interdisciplinary team members (nursing, PT, OT, social work) to ensure holistic and coordinated care.
  • Monitors and documents progress toward established goals and adjust plans as necessary.
Qualifications
  • Master's Degree in Speech-Language Pathology from an accredited program.
  • Current licensure as a Speech-Language Pathologist in the state of practice.
  • Home health experience a plus.
  • Certificate of Clinical Competence (CCC) from the American Speech-Language-Hearing Association (ASHA) required for adult.
  • Valid driver's license, auto insurance, and reliable transportation.
