Job summary

Home Health Care, Inc. is seeking a Speech-Language Pathologist (SLP) to provide in-home therapy as part of a dedicated interdisciplinary team in Minnesota.

This full-time role offers a competitive compensation package and the chance to deliver patient-centered care in patients' homes. You will evaluate and treat communication and swallowing disorders, develop individualized plans, educate caregivers, collaborate with a care team, monitor progress, and ensure timely documentation.

Qualifications

• Strong clinical judgment and patient-centered communication skills.
• Passion for helping patients maximize independence and quality of life.
• Home health experience is preferred but not required.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate and treat speech, language, cognitive-communication, and swallowing disorders in patients' homes.
• Develop individualized treatment plans that support meaningful functional goals.
• Educate caregivers and family members to reinforce therapy between visits.
• Collaborate with physicians, nurses, physical therapists, occupational therapists, and the interdisciplinary care team.
• Monitor patient progress and adjust interventions as appropriate.
• Maintain accurate and timely clinical documentation.
